PLEASE NOTE: Your application is not complete until all necessary documentation has been provided including a valid government issued ID and income verification documents (such as a W2, social security or disability benefits). These documents can be submitted via this form below or dropped off in-person at the Preservation Resource Center located at 923 Tchoupitoulas St., New Orleans, LA 70130.
If your application is denied the homeowner will receive a denial letter mailed to the application address.
